2014 Nomination Form

Nominations must be received by the close of business on Wednesday, March 19. Nominations may be completed by the following online form, or by returning the completed the .PDF nomination form to Lisa Spaeth.

Criteria to be nominated:

• Must be a Naperville Area Chamber Member for at least one year
• Been in business for at least three years
• Privately held
• Five Hundred (500) or less Full-Time Employees: hours worked by all employees divided by forty (40) = FTE
• May be self-nominated
• Previous category winners become eligible three years after award is received
